Offaly Well Connected (Social Prescribing)

Social connections are vital factors in maintaining and promoting physical, emotional, and mental wellbeing. Offaly Well Connected is a holistic, community-based referral service designed to link individuals with local resources and support networks. Based in Tullamore and Birr, this project provides one-to-one consultations aimed at reducing loneliness and social isolation by fostering meaningful involvement within your local community.

  • Anyone over the age of 18.
  • Individuals feeling isolated, stressed, anxious, or lonely.
  • Those who enjoy the company of others and wish to explore community-based options.
  • Referrals are accepted from GPs, primary care professionals, or via self-referral.
  • Health & Wellbeing: Improve your mental and physical health while increasing self-confidence and self-esteem.

  • Connection: Meet new people, receive social support, and become more involved in your local community.

  • Growth: Learn new skills, participate in new activities, and explore volunteering opportunities.

  1. Consultation: You will meet with a Social Prescribing Coordinator to discuss your hobbies, interests, and goals.

  2. Action Plan: The Coordinator works with you to develop a personal action plan tailored to your needs.

  3. Community Linking: We identify specific opportunities—such as Men’s Sheds, walking groups, arts & crafts, or education—and help you link in with them.

  4. Ongoing Support: The Coordinator provides follow-up support to see how you are getting on and help you transition into your new activities.
